• COPD Service – If you have been advised by the surgery to submit a COPD assessment, please use our form

• Learning disability health check
• First Contact Practitioner Service
• Pharmacist
• Social Prescriber offering non-clinical interventions and support. This includes signposting patients to a range of services such as welfare rights advice, employment support, community activities including exercise, mindfulness, yoga, gardening and coffee mornings
